Before attending your appointment a COVID consent form must be filled out. This MUST be done no earlier than 24hours before your appointment and no later than 12hours prior to your appointment. The link to the form will be on your appointment confirmation. 

You will need to fill out a COVID form for EVERY appointment you come to.

Also you will need to fill in a client consultation form before your first every appointment with me, linked below. This will need to be done once and only updated if data changes on it.
